  • Effective June 9, 2026, the AZ&Me program is now accepting applications for BAXFENDY

  • Effective May 1, 2026, the AZ&Me program no longer accepts new patients for FARXIGA and XIGDUO XR
  • Actively enrolled patients will continue to receive assistance for FARXIGA and XIGDUO XR until December 31, 2026
  • Communications regarding this change were sent to currently enrolled patients and their providers in April 2026

  • Why are FARXIGA and XIGDUO XR being removed from AZ&Me Prescription Savings Program?
  • Beginning in April, several generic equivalents for FARXIGA and XIGDUO XR became available. As a result of this broad generic availability, FARXIGA and XIGDUO XR will be removed from the AZ&Me Prescription Savings Program by the end of 2026.
  • How long will patients continue to receive FARXIGA and XIGDUO XR support?
  • Patients currently enrolled in AZ&Me can continue to receive support until December 31, 2026.
  • What is the last day to request a refill?
  • For patients currently enrolled in AZ&Me, the final day to request a refill will be December 15, 2026.
  • Who should I contact for refill requests or program questions?
  • Patients and their Healthcare Providers can get additional program information and manage refill requests by visiting the AZ&Me website: www.azandme.com or they can speak to an AZ&Me representative by calling 1-800-AZandMe (1-800-292-6363), Monday through Friday, 9:00 AM–6:00 PM ET.
  • Are there any exceptions or extensions beyond December 31, 2026?
  • There are no exceptions or extensions. FARXIGA and XIGDUO XR will be fully removed from AZ&Me on December 31, 2026.
  • Where can patients receive assistance with FARXIGA and XIGDUO XR if AZ&Me will no longer provide it to them?
  • Depending on the patient’s situation, the following resources may be able to provide assistance:

PROGRAM UPDATES

  • Program income eligibility requirements will remain at 300% of the Federal Poverty Level (FPL) for Primary and Specialty products
  • Program income eligibility requirements will remain at 500% of FPL for Rare Disease products

  • New Medicare patients who wish to receive assistance through AZ&Me must complete the AZ&Me application process. AZ&Me Applications may be submitted online or via fax
  • Medicare patients currently enrolled in AZ&Me who wish to continue receiving assistance through AZ&Me must complete the annual re-enrollment process. Patients who are Conditionally Approved must provide updated program consent to complete re-enrollment. Additional Requirements for Specialty & Rare Disease Medicare Patients

  • As a program of last resort, AZ&Me will continue to require that new and re-enrolling Specialty and Rare Disease Medicare patients who meet program eligibility requirements and are Conditionally Approved pursue other assistance options including independent nonprofit organizations (INOs) grants before their enrollment can be finalized
  • INO grants can help eligible patients with costs such as copays, premiums, deductibles, and out-of-pocket expenses for their medication(s)
  • For a list of some independent charitable assistance foundations that may be able to assist, patients can visit:
  • If the patient does not qualify for applicable INO assistance or all available funds are closed, the patient must complete the Specialty & Rare Disease Medicare Enrollment Form and submit supporting documentation to AZ&Me
  • Once the completed form and applicable supporting documentation are received and confirmed, the Conditionally Approved patient’s enrollment will be fully approved, and a Final Approval notification will be sent to the patient and their provider

  • If a patient is no longer in need of assistance from AZ&Me, the patient and their provider have three options to withdraw from the program:
    • Use the Sign In button above to access the patient's AZ&Me Dashboard and click the “Withdraw from Program” link under Other Program Tasks
    •  Complete the AZ&Me Withdrawal Form
      • The completed form may be:
    •  Call 1-800-AZandMe (1-800-292-6363), Monday through Friday 9:00 AM-6:00 PM ET, to speak with an AZ&Me Representative

  • To qualify for the AZ&Me program, patients must not have any commercial (private or employer sponsored) insurance or insurance through a government program other than Medicare
  • Commercially insured Specialty & Rare Disease patients who meet all other program eligibility and fall within one of the following coverage categories may request a Reconsideration:
    • Community Collective Cost-Sharing Plan Denial—the patient must provide Notice of Health Insurance Non-coverage letter from the plan dated in the current calendar year
    • Exhausted Product Copay Card Benefit—AZ&Me will confirm that the patient’s copay card has been exhausted
    • Insurance Denial—the patient must provide proof of insurer denial or non-coverage dated in the current calendar year
    • No Prescription Drug Plan Benefit—the patient must provide documentation of plan details/no prescription drug plan dated in the current calendar year
  • To request an Insurance Coverage Reconsideration, the patient’s prescribing physician must complete and sign the AZ&Me Insurance Coverage Reconsideration Form

Frequently Asked Questions - FAQs About the Updates

No, AZ&Me income eligibility requirements will remain the same in 2027

  • Annual adjusted gross household income must be at or below 300% of the Federal Poverty Level (FPL) for Primary and Specialty products (no change)
  • Annual adjusted gross household income must be at or below 500% of the Federal Poverty Level (FPL) for Rare Disease products (no change)

AZ&Me should always be positioned as a program of last resort. As such, Specialty and Rare Disease Medicare patients should exhaust other avenues of assistance, including independent charitable assistance foundations, prior to completing their 2027 enrollment.

While not required, the AZ&Me Digital Assistant allows patients to quickly and easily access real-time enrollment information, submit updated consent (required for all Conditionally Approved patients) and submit proof of denial or fund closure from 3 independent charitable assistance foundations (required for Conditionally Approved Specialty and Rare Disease Medicare patients).
