Eligibility Requirements

To receive support from the AZ&Me Prescription Savings Program, you must meet the following eligibility requirements:

Be a resident of the United States
Be treated by a US-licensed healthcare provider for a medication on AZ&Me. Click HERE to see available medications
Not have any commercial (private or employer-sponsored) insurance or government insurance other than Medicare
- If you are a Medicare Beneficiary, you must not be eligible for or enrolled in Medicare’s Extra Help program, also known as Low-Income Subsidy (LIS) for Medicare Part D. For more information on Medicare's Extra Help (LIS) program, click HERE
Not receiving any other form of medication assistance

Current Program Income Requirement
For Primary and Specialty Medications	Annual adjusted gross income must be at or below 300% of the Federal Poverty Level (FPL)
For Rare Disease Medications (WAINUA)	Annual adjusted gross income must be at or below 500% of the Federal Poverty Level (FPL)

If you have experienced a life-changing event in the past year, you may still be eligible. Some examples of a Change in Circumstance include:

Job loss
Changes in income
Loss of insurance coverage
Change in household size
