Take an Eligibility Assessment

 

To receive support from the AZ&Me Prescription Savings Program, you must meet the following eligibility requirements:

  • Be a resident of the United States
  • Be treated by a US-licensed healthcare provider for a medication on AZ&Me. Click HERE to see available medications
  • Not have any commercial (private or employer-sponsored) insurance or government insurance other than Medicare
    • If you are a Medicare Beneficiary, you must not be eligible for or enrolled in Medicare’s Extra Help program, also known as Low-Income Subsidy (LIS) for Medicare Part D. For more information on Medicare's Extra Help (LIS) program, click HERE
  • Not receiving any other form of medication assistance
Current Program Income Requirement
For Primary and Specialty Medications Annual adjusted gross income must be at or below 300% of the Federal Poverty Level (FPL)
For Rare Disease Medications (WAINUA) Annual adjusted gross income must be at or below 500% of the Federal Poverty Level (FPL)

If you have experienced a life-changing event in the past year, you may still be eligible. Some examples of a Change in Circumstance include:

  • Job loss
  • Changes in income
  • Loss of insurance coverage
  • Change in household size
